UC Davis Health Transitions of Care Department is seeking a Lead Physician and a Staff Physician to join a multispecialty team. The TOC program aims to enhance the experience of a patient’s clinical outcomes by ensuring seamless patient flow as they are discharged from the hospital and connect to their post-discharge clinical and community resources. The TOC SNF Collaborative Program currently has partnerships multiple facilities in Sacramento and co-manages all aspects of care and resources for all TOC-identified patients, including those identified as high utilizers and at high risk for readmission. Candidates must be board certified or board eligible in family or internal medicine with a preference for geriatric fellowship training.
