As a post-acute care medical director and physician, you will be a part of an integrated care team providing primary care services in post-acute care facilities including skilled nursing and assisted living. Our patient population includes long-term care, short-term rehab, memory care, and clinically complex patients and residents. Our physicians are responsible for driving high quality, safe and cost-effective care of patients in our local post-acute care communities. You will work on an interdisciplinary team rounding on all patients as medically necessary as required by CMS. This includes but is not limited to H&P, admission, progress, transfer, discharge, and acute/follow up. You will also work with the facility to transfer patients to the level and site of care most appropriate for their medical needs upon discharge including returning to home, independent living centers, assisted living, and/or memory care. Our clinicians work closely with hospice, palliative care, and home care agencies to ensure a safe transition home. Our clinicians are also responsible for timely documentation and billing through our EMR (Gehrimed) that meets Medicare guidelines. As a TeamHealth post-acute medical director, you will provide oversight, guidance, and mentorship to our clinicians within your facilities. In this role, you will conduct clinical case reviews as needed and share tools, information, and guidelines as they relate to quality of care, represent TeamHealth on appropriate external levels identifying, engaging and establishing/maintaining relationships within the healthcare community, and collaborate with clinicians, hospital leaders and others regarding multidisciplinary care and
